Palpebral malpositions (Ectropion and Entropion)

Palpebral malpositions are alterations in the position and function of the eyelids that can affect ocular health and facial aesthetics. These conditions include ectropion, where the eyelid folds outward causing tearing due to eversion of the lacrimal point, and entropion, which occurs when the eyelid folds inward causing irritation due to damage caused by the eyelashes rubbing against the ocular surface. These malpositions can be congenital or acquired, and their management, mainly surgical, is essential to prevent complications such as ocular dryness, infections and visual problems.

Palpebral ptosis

Palpebral ptosis is another type of eyelid malposition. It is a condition characterized by the abnormal descent of the upper eyelid, which can affect vision and the aesthetic appearance of the face. This alteration can be unilateral or bilateral and can occur at various ages, from birth (congenital ptosis) to adulthood (acquired ptosis). The causes of ptosis are varied, including weakness of the levator muscle of the eyelid, nerve damage, aging or a mechanical cause that produces the descent of the eyelid. In addition to its cosmetic implications, ptosis can interfere with visual function, causing problems such as visual field obstruction and eye fatigue. Timely diagnosis and treatment are crucial to improve both ocular function and the patient’s quality of life.

Epiphora (watery eyes)

Epiphora is a condition characterized by excessive production or inadequate drainage of tears, causing them to spill and overflow over the lower eyelid and cheek. The causes of insufficient drainage may be due to obstructions at different levels of the tear duct, or to functional causes such as a failure to pump the tear along its course. The causes of excessive production may be due to irritation of the ocular surface caused by eyelid malposition, rubbing of the eyelashes against the ocular surface, infections, allergies, among others. Epiphora can not only be uncomfortable, but can also affect the patient’s quality of life, generating discomfort and secondary complications. The treatment of epiphora varies according to its underlying cause. It may include conservative measures, such as the use of artificial tears to relieve irritation, to surgical interventions to correct obstructions or malformations. Accurate identification of the cause is essential to select the most appropriate and effective treatment to restore tear function and improve the patient’s well-being.
